Create a form
Start from a template or a blank form. You can change everything later.
Go to Website › Forms and click Create.
Select a template: Contact, Event, Catering, or Blank. A preview shows on the right.
Click Create.
Enter a Form name, an Introduction, and the text for the Submit button.
Add and arrange fields
Each form is a list of fields. The preview on the right shows what guests will see.
Click Add field.
Choose a field type, then click Add.
Drag fields to change their order. Select a field to edit it.
Field types come in 3 groups:
Contact information: the guest's details, like name, email, phone, address, and location (Location Picker).
Questions: Text Field, Date & Time, Single Choice, and Multiple Choice.
Content: Heading and Paragraph, to organize your form and add instructions.
Publish your form and add it to a page
Your changes aren't live until you publish. A new form isn't on your website until you add it to a page.
Click Publish, then confirm. This updates the form everywhere it's used.
To put a new form on your website, click Add to Website. Your website Pages open.
Open the page you want, add a section, and choose Form.
Select your form, then save the page.
You can add a form to a page anytime from Website › Pages. You can use the same form on more than one page.
ℹ️ Note: If you leave the editor before publishing, you'll be asked whether to discard your unpublished changes.
View and export responses
Go to Website › Forms and click the form. Its Submissions list opens.
Click a response to see all of the guest's answers. From there, you can Mark Unread.
To download all responses as a CSV file, click Export.
✅ Tip: Click View Form to edit the form from its Submissions page.
Delete a form
⚠️ Deleting a form also deletes all of its responses. This cannot be undone. Export them first if you need to keep them.
Go to Website › Forms.
Check the Status column. It shows which pages use the form, or Not used.
Click the trash icon next to the form, then click Delete.
Choose who gets form responses
Everyone on your team who can view form submissions gets an email and a notification for each response. Each person can turn these off with the Website forms option in their notification settings.
To also send responses to people who aren't users, like a catering manager, add their email at Brand settings › Alerts › Form submissions.
Reply directly to a guest
When you get a form response email, reply to it directly. The guest's email is the reply-to address, so your reply goes straight to them. This doesn't apply to review or feedback emails, which reply to Owner Support.
✅ Tip: Form responses also show up in your Inbox, where you can reply by email or text.
Send responses to the right location
If you have more than one location, you can send each response to that location's team. Single-location brands can skip this section.
Go to Settings › Location Settings › Notifications › Form submissions, and add the people for each location.
Add a Location Picker field to your form.
Who else gets each response:
If your form has a Location Picker, responses also go to the people set for the location the guest picks.
If it doesn't, responses also go to the people set at Brand settings › Alerts › Form submissions.
